中藥學堂 同化製劑與利尿劑篇 中藥運動禁藥補益與代謝的陷阱(二) ◎文/藥師楊佳蒨 利尿劑則能增加尿量和鈉的排泄,改善體液的組成和容量,並清除組織中多餘的液體。在體育運動中常被濫用於稀釋尿液中的興奮劑及其代謝物,增加反興奮劑檢測的難度。另外在有快速減重需求的運動員中濫用情況也十分普遍。特別是在有體重分級或運動員希望保持較低體重的項目中。1 實證文獻顯示中藥單複方,如茯苓、豬苓、澤瀉、車前子、夏至草、芫花、五苓散及當歸芍藥散等,均含有與世界反運動禁藥機構(WADA)禁用的利尿劑相似的成分或效用。 茯苓(Poria)和豬苓(Polyporus):經動物實驗證實含有多種促進排尿的成分(如ergosterol、D-mannitol),其作用機制與臨床利尿劑相似,主要透過腎臟調節水分與電解質排出。13,14 澤瀉(Alismatis Rhizoma):含有利尿成分,特別是乙酸乙酯和正丁醇萃取物等成分,在動物模型中可增加尿量和電解質排泄。15,16,18,20 車前子(Plantaginis Semen):在動物實驗中,無論是生品或鹽製品,均能顯著增加尿量與尿中電解質排出,並降低腎臟醛固酮與水通道蛋白AQP2表現,顯示其利尿機制與臨床利尿劑相似。14,17 夏至草(Lagopsis supina (Steph.) Ikonn.-Gal. ex Knorr.):所含酚丙烷類(Phenylpropanoids)/黃酮類(flavonoids)等水溶性成分可抑制腎臟水通道蛋白(AQP)及腎素-血管收縮素-醛固酮系統(Renin-Angiotensin-Aldosterone System, RAAS),產生急性與持續性利尿作用,且不易造成電解質異常。18 參考資料: 3.
